Hyderabad, 10th May, 2021: A rare paediatric liver transplant has been performed on a 3-year-old boy at CARE Hospitals, Banjara Hills, Hyderabad. Liver transplantation is a surgery requiring a specialized team of doctors and a highly sophisticated setup.

 

Master Rocky Das (name changed) is a 3-year-old baby who was suffering from end-stage liver failure due to Caroli’s disease. He was grossly malnourished and frail, weighing only 8 kg. His family, hailing from Kolkata, had taken him to multiple hospitals across the country for treatment. He was suffering from deep jaundice, recurrent fever, huge water collection in the tummy and frequent black stools due to bleeding in the stomach. He had been advised liver transplant surgery, but owing to lack of blood group compatible liver donor, poor general condition and financial constraints; all the hospitals they visited had refused to do the surgery. 

 

“Caroli’s syndrome is a rare genetic condition that affects about one child in a million. In this condition, the bile ducts in the liver are hugely dilated. Widening of the bile ducts both within and outside the liver can cause stones to form in the bile duct and liver, which can lead to jaundice, recurrent fever, blood vomiting, black stools, liver fibrosis and high blood pressure in the portal vein. These changes happening over a period of time can cause the liver to get permanently damaged leading to chronic liver failure.”says Dr Mohammed AbdunNayeem, Clinical Director and HOD, Care Institute of Digestive Diseases and Liver Transplant, CARE Hospitals, Banjara Hills. 

 

Liver transplant surgeons at CARE Hospitals, Banjara Hills offered to do a rare “blood group incompatible liver transplant” for the child. As the child’s blood group was not compatible with either of the parents who were willing to donate a portion of the liver, the child needed to undergo a blood group incompatible Liver Transplant with the father chosen as the donor.

 

Dr Nayeem added “ABO incompatible liver transplants are rarely performed on children. Very strong medicines are administered days before the surgery to suppress the child’s immunity. The child’s blood is then purified through plasma exchange to help accommodate the incompatible liver. All this significantly increases the kid’s chances of becoming sick with infection.”To add to the complexity of the surgery, there was an under developed portal vein along with dilated and damaged bile ducts. To provide a good blood flow to the newly transplanted liver a new portal vein had to be reconstructed using a blood vessel graft from a deceased donor.

 

As the family hailed from a very poor socioeconomic background and could not afford to undergo all this treatment before the surgery and the transplant cost, the hospital arranged the full finances for the pre-operative optimization and the complicated surgery via a crowd-funding platform. 

 

The child was discharged 10 days after the successful surgery.“We are happy to report that the child and the donor (father) are both doing fine. They visited us again today for a follow-up and things seemed normal. He will be able to lead a normal and healthy life soon,”said Dr Vachan, Consultant Liver Transplant surgeon, CARE Hospitals, Banjara Hills, Hyderabad.
