Before getting into the deeper aspects, let us first understand the basic anatomy of our shoulder. In a broader perspective, our shoulder is a holistic combination of three bones, Humerus (upper arm bone), Scapula (shoulder blade) and Clavicle (collarbone). A group of four muscles called rotator cuff surrounds the shoulder joint to protect it and is a very potential source of tear and pain from the shoulder joint. It is the disruption and disturbance created in these bones and muscles that results into pain. If this pain persists for more than 6 months it is called Chronic Pain. Besides physical injuries, majority of the shoulder pain is caused by,
Rapid urbanization, lack of physical activity and sedentary lifestyle are few of the significant factors that leads to above mentioned disorders. Though they look trivial in the initial stage, if ignored for long periods, they can create severe damage to muscle and joints. In majority of the cases, patients usually don’t even remember and know the exact reason behind their pain. They seek medical attention only when the situation become adverse.
Patients with Osteoarthritis are usually advised surgical treatments. Owing to painful recovery process, large incisions, lengthy healing process and prolonged hospital stays, patients feel reluctant to undergo the treatment. Furthermore, these procedures do not actually get to the root cause and deliver comprehensive treatment. Rotator cuff muscle injury especially the supraspinatous muscle can be strained, partially or completely torned. Most of the time , such tears cannot be healed or treated. Regenerative therapy is a revolutionary non-surgical alternative procedure which is no less than a boon to the patients. This procedure helps in regeneration of degenerated cartilages, tendons and muscles which has occurred due to ageing, obesity, trauma, diabetes and other clinical complications.
PLATELET-RICH PLASMA (PRP)
The process is very simple and done as a day care procedure.
Patient's own blood is taken using a thin needle and blood is transferred in a special container called cartridge and this cartridge is put into a particular machine tor centrifugation. After the centrifugation blood is separated into two different layers. The lower layer is the RBC and top most layers are called the plasma. These two layers are separated by closing a channel between them present in the cartridge. After this another round of spin is done which will separate 2ml of platelet rich plasma from rest of the plasma. This platelet rich plasma is rich in growth factors. The entire processing of preparation takes 20-30 minutes. The preparation for PRP is done in a specialized machine under appropriate temperatures. After the PRP is prepared, the patient is taken into the operation theatre. The torn area is identified under ultrasound or fluoroscopic machine and the PRP containing the growth factor prepared is injected into the damaged area.
Once the PRP is injected into the damaged area, it creates a small amount of inflammation that triggers the healing cascade. There are number of growth factors and enzymes released which will lead to formation of collagen and thereby repair and regenerate the tissue. It takes only 3 months for the entire tissue to regenerate again.
Rotator cuff injury will require only one sitting of PRP therapy and few other conditions like shoulder osteoarthritis will require three sittings for the complete regeneration which are done at an interval of 15 days.
