It is indeed the core essence of our profession. It is no less than a blessing to be able to serve people, alleviate their ailments and impart health in their lives. Being a Doctor, it gives me immense joy to witness the happiness of my patients after their recovery. This is a tale of one such patient who had become like a family member to me within a very short span of time. The first and wakeup call on every festival day will be always from him.

 

This goes down back to 16 years, when this guy was treated for Endoscopic Cystogastrostomy following Pancreatitis and he developed Pancreatic Ascites with Pancreatic Necrosis & multi-organ failure. He was in dire need of treatment and he was referred to me for surgical management of Pancreatic Ascites and Sepsis. He underwent Pancreatic Necrosectomy and he had a critical one month period to sustain and overcome the fatalities. He was accompanied with his wife who was 9 months pregnant and due for delivery anytime. He was critical and on ventilator and I had to say that he is ok and he will recover soon so that he stay confident and be hopeful for a better tomorrow. In reality he developed lot of complications with Pancreatic Fistula with bleed from the Fistula and Retroperitoneal collections and so on. It was a stormy course and at end of the storm he recovered completely. His wife delivered a beautiful boy during this critical phase. His family was whole and complete. He was in touch for follow-up monitoring and medications whenever needed. He was leading a normal life.

 

As years passed, he used to talk a lot about his naughty son, he used share his happy moments. His every call gave me a befitting confidence that every critical case will have a happy ending and what is necessary is to work on them with sincerity and commitment. You will really feel worthy of your profession.

 

But those cherishing moments and blissful days, didn’t last forever. One morning, about 10 days back (as on 3rd Sep, 2020), I got a call that he is having Pedal Edema, Mild fever and weakness. I asked him to consult a local physician and check for Covid. At 2:00 pm I got a call that he is positive on CT scan. I asked him to get admitted locally or travel to nearby Bangalore. His wife said she prefers to travel to Hyderabad. I made the necessary arrangements in Hyderabad. He reached at 8.00 pm to Emergency.

 

I got a call from emergency that he is critical and at low saturation levels. He should be admitted into ICU but unfortunately at that moment we didn’t had any vacant beds. There were frantic calls from my side to everyone to arrange an ICU bed. Due to non-availability I had to call all hospitals known to me, the response was they can only arrange the next day and not immediately. I made a request to emergency physician that I will take responsibility for any events but he will not be left out of hospital. I made calls to my Medical Director to give permission to keep him in Emergency for that night. He was considerate and agreed to my request. As I was thinking that let the night pass by, I got another shock. His creatinine is more than 12 and he requires an emergency management. At this juncture I requested my Nephrologist to see the patient. Then I was Lucky My nephrologist took up the case from me and arranged for an emergency dialysis at night.

 

The dialysis was done every day and lung care was given. He improved very well and the next four days all were happy for a good outcome. Then I got a call on Sunday that he is suddenly unconscious and may be had a stroke. MRI brain was normal. The reason was Covid induced sepsis. Then he had a bad course where he was not responding to medications and on Monday two days back he was no more. I did not have answers to his wife, she came all the way but I could not help him. Covid has literally taken him away from us. What I can say that is inspite of the best facilities and medications, I could not save him. I am not going to have my morning greetings anymore. He smile and Happiness which I am going to miss forever. I am lost in the thoughts what has this Covid pandemic done to all of us and changed our lives so fast and it’s never going to be same again.

 

My mornings will not have his happy greetings anymore.
