COVID VS NON COVID PATIENTS HOW TO BALANCE AND PROVIDE HOLISTIC HEALTHCARE FOR EVERYONE

Amidst the chaos of fatal pandemic there is also a parallel threat of misinformation being circulated in large numbers. It is the responsibility of health professionals and concerned authorities to disseminate myths and impart scientific and ethical knowledge to the society. We cannot put on hold and keep waiting for the Covid-19 to subside and ignore patients who are suffering with chronic illnesses like Cardiac problems, Renal Diseases, Brain Tumours, Cancer etc. We have been focussing more on Corona even after the lockdown. The crude death rate in other diseases has been increased significantly. The estimated number of deaths in the past 6 months is 25 Lakhs. Non-Covid-19 Patients are paying the price of India’s efforts against the Coronavirus. At least 25% of the deaths could have been easily prevented if we had the service to take care of Non-covid Patients.

 

Lockdown has been implemented to get prepared and develop the required health infrastructure so that we can treat the increasing number of Covid patients. But during this period, we haven’t attended people who are suffering with other diseases. Now, with all the piled up pending cases and the new Covid cases, the total load on the health industry is doubled up. How do we tackle with this? It is the biggest challenge. As per a publication by Harvard University, Covid will persist till 2022. We cannot Lockdown for entire lifetime, we need to coup up and bridge the gap. We need to implement strategies where we can manage both Covid and Non-covid patients. There are few studies which suggest Covid test to be done before treatment in elective cases. If the patient is Covid positive then we first treat for Covid, wait for 1 month and then perform the required treatment of other disease. The risk of lung complications is high if we operate on a Covid positive patient.  Hence it is important to perform Covid test in all patients where surgery is planned. ADVANTAGES OF PRE-OPERATIVE COVID TESTING

  • Protects the healthcare workers
  • Protects the fellow patients & their relatives

 

The irony here is, Covid testing is done in only 15% of the people because others have very mild symptoms or asymptomatic. The 85% people who come for OP may or may not be infected with Coronavirus, we cannot ascertain it. Healthcare professionals are always at risk irrespective of the current condition of the patient. We must take proper measures such N95 Masks, Single Piece Biological Protection Suits etc.

 

The principal problem with the suits is they cannot be worn for more than 3-4 hours. Astronaut suits have inbuilt oxygen cylinders in them so they can be worn for 24 hours whereas Biological Protection Suits with oxygen supply are very expensive ranging upto INR 5 Lakhs. They are not feasible not just in India but in western countries as well. The only solution is to complete surgeries within 3-4 hours or involve more surgeons and divide the work. It is not an easy task to treat both Covid and Non-covid patients. The chances of getting infected is 10-15% if a doctor is treating a Covid patient. So, if a hospital intends to continue their operations they need to double their personnel so that they can work in cycle like work for 1 week and go for quarantine next week. Besides procuring equipment, PPE kits, Ventilators, governments should focus more on recruiting more number of healthcare workers. It is indeed the need of the hour.

 

The proposed model to run a hospital is to divide exactly 50% of the beds for Covid & Non-covid patients. If 1X personnel is required to manage 50% of Non-covid beds, we need 2X to manage other 50% of Covid beds. If we increase the team size, the existing health infrastructure is sufficient and adequate to meet increasing cases of both Covid & Non-covid patients.
