Deadly clots are seen more often in second wave of Covid infection

(A timely referral to a vascular surgeon can save life and limb)

33 yrs Ramesh (name changed) was recovering from Covid infection, suddenly started complaining of severe pain in his both legs and left hand. The condition became further worse when he started loosing power in the limbs and it started turning pale and blue. He was then referred to higher center.


There was no blood supply to his both legs and left hand and was almost reached to irreversible state of amputation of both legs and hand. He was immediately taken up for an emergency surgery and these blood clots were removed from both legs and hand. Blood circulation re-started and Ramesh was very lucky to have his both legs and hand saved with intact normal functions. “He was indeed very lucky to get back his blood supply to normal, but there are few cases who referred late are still struggling for the good outcome,” Dr Devender Singh, Senior Vascular & Endovascular Surgeon, Yashoda Hospitals.

 

He says, in this second wave of Covid we are seeing lots of blood clotting cases involving every part of the body, lungs, heart, limbs, brain, stomach, intestine, which requires urgent attention. “Last year we operated such 36 cases, but in this second wave we have already operated 12 such cases in two weeks, most of them referred from other hospital,” he states.

 


Covid infection is a vascular disease, the virus damages the blood vessels of any part of the body, most commonly lungs, leading to clotting(thrombosis) of blood vessels which causes sudden death. The virus attacks both the venous and arterial system of the body leading to poor outcome. “A clot in the vein is known as deep vein thrombosis (DVT), which can travel to lung, a condition known as pulmonary thromboembolism (PTE), which is a cause for sudden death. A clot in the artery will stop the blood supply to that organ, causing death of the part if not attended on emergency basis. A clot in the carotid artery can leads to a stroke which can be fatal or associated with disability. A clot in the legs and hands, if not treated urgently can cause limb loss(amputation).A clot in the intestine can kill the patients with infection in no time.”

Diabetic, smoker, hypertensive, kidney failure and obese patients are more prone for the sudden clotting in the blood vessels in Covid infection. These patients carries a very high risk and should be monitored carefully for the occurance of blood clotting.“In the blood investigation Increasing D-dimer, platelets, fibrinogen, indicates high risk for blood clotting and so the physician should be careful  and these patients to be considered for full dose of anticoagulation(blood clot dissolving/ blood thinner medication).”


A timely referral to a vascular surgeon can save life and limb in these patients. “Early detection of these clots, can be managed with simpler procedure like embolectomy(removal of clots), which can be done under local anaesthesia, sometime bed side in very sick patients. Percutenous procedures and thrombolysis is also an option in few cases.”


Patients with moderate to severe covid infection should be discharged on anticoaglution to avoid these catastrophic events of limb loss and permanent disability.
