29th December, 2021: It was indeed a cheerful moment at Malla Reddy Narayana Multispeciality Hospital (MRNMH). The Department of Cardiology has successfully performed a very rare Transcatheter Aortic Valve Replacement (TAVR) on a 80-yearold female. In view of patient’s age & other clinical complexities, it was a phenomenal achievement. The surgery was jointly performed by Dr. Kamal Kumar Chawla, Consultant Cardiologist, MRNMH & Dr. Prakash Ajmeera, Consultant Cardiologist, MRNMH. Giving his note on the occasion, Dr. Kamal Kumar Chawla, said, “Mrs Kausalya (aged 80 Years) presented at Hospital with complain of severe breathlessness for past 3 days. After evaluation, she was diagnosed as accelerated hypertension, severe Calcific AS with acute LVF, NYHA-IV.

She was admitted for further management. After stabilization, she was advised for Aortic Valve replacement, considering her age and other morbidities. She underwent the surgery and discharged with hemodynamically stable condition. The development of Transcatheter Aortic Valve Replacement (TAVR) has emerged as a lifeline for patients considered to be inoperable providing both improvement in symptoms and statistically significant mortality benefit.” Transcatheter aortic valve replacement (TAVR) is a minimally invasive heart procedure to replace a thickened aortic valve that can't fully open (aortic valve stenosis).

The aortic valve is located between the left lower heart chamber (left ventricle) and the body's main artery (aorta). If the valve doesn't open correctly, blood flow from the heart to the body is reduced. TAVR can help restore blood flow 1-1-216, Suraram'X' Roads, Jeedimetla, Hyderabad, Telangana 500055 and reduce the signs and symptoms of aortic valve stenosis — such as chest pain, shortness of breath, fainting and fatigue. During this minimally invasive procedure a new valve is inserted without removing the old, damaged valve. The new valve is placed inside the diseased valve. Usually valve replacement requires an open-heart procedure with a “sternotomy”, in which the chest is surgically separated (opened) for the procedure.

The TAVR or TAVI procedures can be done through very small openings that leave all the chest bones in place. The procedure is available to patients in all risk categories. TAVR can be an effective option to improve quality of life in patients who otherwise have limited choices for repair of their aortic valve. Malla Reddy Narayana Multispeciality Hospital has always been at the fore front in performing challenging cases and saving lives of individuals during critical times. This rare surgery is a definite milestone accomplishment which has set the benchmark standards.
