COMPLEX PINHOLE SURGERY PERFORMED THROUGH WRIST AT STAR HOSPITALS TO TREAT A RARE BRAIN DISORDER

The Department of Neuroimaging and Interventional Radiology of Neurosciences team at STAR Hospitals performed a complex Neuroendovascular Intervention -Pinhole Surgery for blood vessel disorders of brain, through a small artery in the wrist-radial artery.

 

Here are the excerpts from the insightful interview with Dr. Shahyan Siddiqui, Neuro and Interventional Radiologist at STAR Hospitals, who performed the Surgery.

 

Q. Can you please give us a brief overview of the case?

 

A. The patient was a 55-Year-Old male from Hyderabad,a known case of chronic kidney disease ,was referred to the Neurology department with the complaints of difficulty in talking and seizures. He underwent an MRI scan of the brain that showed edema (swelling) of the brain tissue. Further, an angiogram of the blood vessels of the brain confirmed the diagnosis of Dural Arteriovenous Fistula.

 

 

Q. What is Dural Arteriovenous Fistula?

 

A. It’s a rare blood vessel disorder of brain (incidence of 0.15 to 0.29 per 100,000 persons per year). In this condition an abnormal connection between arteries and veins of the brain called “fistula” is formed. This causes swelling of the brain tissue that can manifest as headaches, seizures and even life-threatening Haemorrhage within the brain if left untreated.

 

Q. What was your clinical approach?

 

A. As the angiogram also was done through the radial artery, we decided to treat the condition through same route. The abnormal connection called “fistula” was closed by navigating tiny tubes called microcatheter and hair thin microwires into the abnormal arterial and venous connections and injecting liquid embolic agent that sealed them instantly.

 

Q. Why this surgery was performed through wrist?

 

A. The traditional pin hole surgeries done through the artery of the groin (femoral artery), where patient has to be immobilized for at least 12 hours, a urinary catheter is placed, causing lot of discomfort to the patient. As Neuroendovascular intervention performed though transradial route enhances patient's comfort, safety, shorter hospital stay over an open brain surgery. The patient could be mobilized within a few hours. He could sit in the bed and perform his basic activities without any hindrance.

  • Dr. Shahyan Siddiqui completed his M.B.B.S., and M.D. from Dr.NTR University of Health Sciences. He further underwent his D.M. (Neuroimaging and Interventional Radiology) superspecialisation at the National Institute of Mental Health and Neurosciences, (NIMHANS) Bengaluru. A Quartiary referral centre in Asia.
  • Here he was trained in all aspect of diagnostic neuroradiology complex neurovascular and pain interventions in addition to peripheral vascular, body interventions.
